Advance statements in old age psychiatry

Lisa Williams and John Rigby

Lisa Williams is a specialist registrar in old age psychiatry and is based at The Hesketh Centre (Merseycare NHS Trust, 51–55 Albert Road, Southport, Merseyside PR9 0LT, UK. E-mail (secretary): Carol.Rotherman{at}nswl-tr.nwest.nhs.uk). Her particular interests are in young-onset dementias and medical ethics. John Rigby is a consultant psychiatrist at The Hesketh Centre. He has a special interest in medical education.

Advance statements, originally designed for situations of terminal illness, are becoming increasingly relevant in psychiatry. This article examines their background, rationale and use in old age psychiatry. The difference between advance statements and advance directives is highlighted, and different roles are suggested for each. We discuss the advantages, and the possible limitations, of advance statements, including the unique issue of the Mental Health Act 1983.
