Being a trainee psychiatrist in the Department of Psychiatry Aga Khan University Hospital Karachi, I am delighted to know that the Royal College constantly revises its psychiatric training curriculum to meet mental health challenges. Many of our faculty members are also Royal College members, so that such sweeping changes in the Royal College psychiatry curriculum also interest us. In The Aga Khan University Hospital , the Post Graduate Medical Education Department (PGMED) in collaboration with the Departmental Residency Committee(DRC) has devised a structured and organized teaching curriculum and revises it each year. In addition to that, an annual summative review(ASR) takes place during which trainees are individually given feed back by the faculty in the light of assessments during the previous year. In first year of training PGMED arranges an extensive series of didactic lectures and workshops with the objective of covering clinical and service related areas, research methodology and bioethics. These are mandatory for all residents irrespective of their speciality. In the second year psychiatry trainees attend a one month course on biostatistics and data management, sponsored by Department of Psychiatry and conducted by the Community Health Sciences(CHS) faculty the university. Trainees in almost all clinical disciplines are registered with the College of Physicians and Surgeons Pakistan(CPSP), which is an examining body with its own prospectus and statutes. I am wondering if the Royal College of Psychiatrists could liaise with our department through our university PGME to establish a new examination centre for the MRCPsych. |

This editorial by Professor Dinesh Bhugra is excellent in that it gives trainees an idea of what the future expects from them. It explains clearly the benefits of work place based assessments when compared with current methods of training. But in practice some consultants already find they have very little time for supervision. It will be interesting to see how far workplace based assessments ,some of which need a supervising consultant sitting with the trainee throughout a clinical interview, will be feasible. The e-letter submitted by A-Wahab Yousafzai suggests holding the Royal College of Psychiatrists membership examinations in other countries. I think this is an excellent idea, but it would only be beneficial to the candidates if both the written papers and the clinical exams are held abroad, rather than only the written parts. This might have avoided the problem a few years ago when candidates who passed their written examination in Hong Kong were unable to come to UK to complete their clinical exam because of the Asian epidemic of bird 'flu.
